Please bring your insurance card and photo identification. Your therapist will make copies of these.
Our therapist will discuss your goals for therapy with you. You will also have a few brief assessments to complete. Information will be gathered by your therapist, such as a brief history, your symptoms and other information relevant to your mental health.
Finally, your therapist will discuss treatment options with you in preparation for later treatment planning.
Please wait in your car or outside the building until your appointment time. When entering the building, use alcohol-based hand sanitizer or wash your hands.
Wearing a mask is required in all common spaces; individual may discuss any in-session needs with your therapist. If there is nobody to greet you when you arrive, please wait for your therapist in the waiting area.
We will keep a distance of at least 6 feet and avoid unnecessary physical contact (no shaking hands, hugs).
A legal guardian must be present for the first two sessions of any minor client. Parents of children under 16 are expected to stay on the premises.
Parents of clients ages 16 and older must first make arrangements with the therapist to be off-site.
